Hospice Home Health Aide

About the role

Suncrest Hospice is seeking caring and compassionate Hospice Aides to join our team. This position will travel throughout our territory, providing care for a group of hospice patients where they reside, which could include private homes and skilled nursing facilities. At Suncrest Hospice our goal is to change the expectation of hospice care in your area by providing exceptional care and service to our patients. This is achieved by allocating the resources to increase our staff-to-patient ratios, thereby increasing clinical visits while lowering clinician caseloads. We are proud to be a Community Health Accreditation Partner (CHAP) certified hospice.

Responsibilities

• Work under the direction of a Case Manager and provide resident care to assigned residents in accordance with resident assessments, care plans, and as directed by nursing management
• Work with other licensed care staff to coordinate the proper types of care to residents
• Strictly follow all Company nursing policies and procedures as well as safety guidelines
• Work closely with patients on a 1-1 level while getting strong support from a patient care team
• Collaborate daily with nurses, social workers, chaplains, volunteers, and HHA leadership

Qualifications

• HHA certification in the state of New Jersey required
• CPR certified
• Valid driver's license and auto insurance
• Reliable transportation
• Ability to work independently on a day-to-day basis
• Ability to work well within a team environment
• Strong commitment to improving quality of life while partnering with community organizations in patient care

Pay

$20-$24 per hour + bonus opportunities.
