About the role
The primary function is to provide healthcare and related services to the patient in their place of residence; assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor.
Responsibilities
- Promote hospice mission, goals and philosophy.
- Demonstrate commitment, professional growth and competency.
- Demonstrate compliance with policies, procedures, regulations defining scope of practice.
- Promote security and safety of Protected Health Information.
- Participate in programs and on committees as requested.
- Participate in ongoing education as required.
- Provide services according to the Hospice Aide Care Plan.
- Carefully coordinate with RN-develop/update plan of care to meet the needs.
- Attend IDG meetings, as requested.
- Document services and submit timely.
- Report changes to the RN.
Qualifications
- A State Driver’s License.
- At least one year preferred, as a hospice aide or nursing assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
- Strong communication and interpersonal skills.
- Sensitivity to issues of loss and grief.
- Ability to deal effectively with the demands of the job and population serviced.
- Reliable transportation.
- CPR certified.
- TB health screen or TB test/CXR according to policy.
- Hepatitis B Vaccination Consent/Declination.
