At Brighton/Suncrest Hospice, our goal is to change the expectation of hospice care in your area by providing exceptional care and service to our patients. This is achieved by allocating resources to increase staff-to-patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Responsibilities

  • Provide care and services for a group of patients, visiting them where they reside, including private homes, Skilled Nursing Facilities, and assisted living facilities.
  • Serve as a contributing member of the interdisciplinary team, offering expertise in psycho-social assessments, interventions, and knowledge of community resources.
  • Work in either a Case Management, Admissions, or Resource role.

Qualifications

  • Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.

Skills

  • Works well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Able to work closely with patients on a 1-1 level while receiving strong support from a patient care team.
  • Can work independently on a day-to-day basis, managing activities such as phone, email, and written communication, meetings with healthcare professionals, and understanding hospice rules and regulations.
  • Possesses time management skills to manage visits, documentation, and meetings, including prioritizing and triaging urgent needs.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.

Schedule

Full-Time, Monday - Friday, 10am-7pm

Pay

$90,000-$97,000 / Salary
