Hospice Director of Nursing

About the Role

The Director of Clinical Services is responsible for the overall direction of hospice clinical services. This role ensures the quality of patient care aligns with the Agency's objectives and policies through planning, coordination, implementation, and evaluation of hospice services provided to patients facing end-of-life healthcare needs.

Responsibilities

• Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
• Provides supervision and guidance to clinical staff to ensure quality of services and maintenance of standards.
• Performs chart audits and gathers data for Quality Assurance and Performance Improvement (QAPI) activities.
• Evaluates clinical staff performance during probationary periods and annually, providing teaching and counseling to enhance service delivery and professional development.
• Identifies and evaluates patient needs and appropriateness of home care by reviewing initial intake screening forms before assigning direct care staff.
• Assists with the orientation of new staff and conducts in-services for current staff.
• Visits patients in the home setting as needed.
• Participates in audits, Interdisciplinary Group (IDG) meetings, and QAPI activities.
• Maintains adequate and appropriate medical supplies.
• Ensures compliance with Medicare, State, and CHAP regulations by all clinical staff.
• Assumes responsibility for self-development through continuing education, workshops, conferences, and active participation in professional organizations.

Requirements

• Graduate of an accredited School of Nursing.
• Registered Nurse licensed by the state.
• Minimum of 2 years of nursing experience in home hospice or skilled home care (hospice experience preferred).
• Ability to market services and deal tactfully with customers and the community.
• Excellent assessment and problem-solving skills.
• Strong verbal and written communication skills.
• Demonstrates self-confidence, a positive attitude, and a commitment to goal achievement.
