About the role
As a Hospice Coordinator, you will play a vital role in providing support, empathy, and guidance to patients and their loved ones during this challenging time. You will coordinate care plans, collaborate with interdisciplinary teams, and ensure the highest quality of care for hospice patients. This onsite role allows you to directly impact the lives of patients and their families every day.
You will work closely with Hospice and Long-term care facilities to ensure patients receive the necessary medical equipment for their unique needs. Serving as a liaison between these facilities and insurance providers, you will verify coverage, navigate the insurance process, and ensure timely delivery of vital equipment for patient comfort and care.
Responsibilities
