Hospice Community Liaison

Responsibilities

• Develop and maintain referral relationships throughout Bergen County
• Educate referral sources on hospice services and patient eligibility
• Grow census through strategic outreach and territory management
• Partner with hospitals, skilled nursing facilities, long-term care facilities, and community organizations
• Meet and exceed established referral and admission goals
• Maintain accurate activity tracking and territory planning

Requirements

• Minimum 2 years of outside sales experience in hospice, home health, or senior living
• Established book of business within Bergen County
• Strong relationships with case managers, social workers, discharge planners, and care coordinators
• Experience calling on hospitals, SNFs, and long-term care facilities
• Proven ability to generate referrals and consistently achieve sales goals
• Excellent communication, relationship-building, and territory management skills

Compensation & Benefits

• Base Salary: $85,000+
• Uncapped Monthly Bonus
