Hospice CNA Field (Home Team)

About the role

Suncoast Hospice, a member of Empath Health, is seeking a caring and motivated CNA to provide personal care and assistance with activities of daily living for hospice patients in their homes in Largo, Indian Rocks Beach, Madeira Beach, and Bay Pines area. Since 1977, Suncoast Hospice has served neighbors across Pinellas and Hillsborough counties with compassionate end‑of‑life care, helping those with advanced illness find comfort, dignity, and meaning while supporting families and caregivers.

Responsibilities

• Provide hands‑on personal care and assistance with activities of daily living.
• Assist patients with taking medications that are ordinarily self‑administered and have been pre‑poured as designated in the Plan of Care.
• Report changes in the patient's medical, nursing, rehabilitative, and social needs to a registered nurse, documenting the changes and to whom they were reported.
• Following a patient’s death, collaborate with the primary RN/Counselor and participate as needed in closure contact with family/caregivers.

Requirements

• One to three months of related experience and/or training preferred.
• Valid Florida Board of Nursing certification (CNA License).
• Valid Florida State Driver’s License.
• Reliable transportation to perform tasks in a timely manner.
• Proof of valid automobile insurance (copy placed in HR file).

Schedule

Monday – Friday, 8 am to 5 pm.
