Hospice CNA

About the role

Cherish-Hospice is seeking an on the road CNA Hospice Aide in Kokomo and surrounding counties. As a hospice aide you can help enhance the quality of life for the patient by providing personal care and related assigned services to hospice patients directly.

Responsibilities

• Provide personal care according to the patient's needs
• Carry out the duties assigned to increase the patient's level of comfort, and to maintain personal hygiene and a safe environment for the patient
• Care provided is in accordance with an established plan of care and under the supervision of an RN
• Observe the patient, report observations and document observations and care performed
• Document the care provided and the patient's response to care in accordance with policies and procedures and regulatory standards
• Provide timely documentation of patient services based on plan of care
• Adhere to Company policies and procedures and regulatory compliance requirements and directives

Requirements

• Travel as necessary (up to 50% of the time) to service patients in their residence of care, which includes home, assisted living facilities, long term care facility, and hospital
• CNA License (Required)
• Driver's License (Required)
• Hospice experience (Preferred)
• Geriatrics experience (Preferred)
• Hospice and Palliative Medicine (Preferred)
