About the role

The Care Team Hospice is seeking a Certified Nursing Assistant (CNA) to provide personal care and comfort-focused support to hospice patients in their homes and facility settings.

Responsibilities

• Provide personal care, hygiene, and comfort measures
• Observe and report changes in patient condition
• Support patients and families with compassion and respect
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase)

Requirements

• Active CNA certification
• Hospice aide experience preferred
• Experience using HCHB (Homecare Homebase) preferred

Qualifications

• Active driver’s license
• Pass a criminal background check
• Pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role

Schedule

Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM, Lehigh Valley, PA and up to a one-hour radius surrounding.
