Hospice CNA - 1st Shift

About the role

By Your Side Hospice is a locally owned hospice provider serving Milwaukee, Waukesha, Ozaukee, Washington, and Racine counties. The organization offers a community‑based, niche hospice focused on patients, families, and caregivers. The Certified Nursing Assistant (CNA) will travel to patients’ homes to deliver hospice and palliative care in accordance with each patient’s plan of care. Responsibilities

• Assist patients with Activities of Daily Living (ADL) such as oral and personal hygiene, bathing, shampooing, skin and nail care, dressing, feeding, elimination, ambulation, repositioning, and linen changes.
• Offer emotional support and encourage family participation in care.
• May assist nurses with treatments or patient assessments.
• Use DME equipment and supplies safely and properly.
• Maintain a safe and healthy environment.
• Take and record accurate patient vital signs or weight when advised.
• Follow the Home Health Aide Care Plan as written, approved, and supervised by the Registered Nurse.
• Observe patients’ physical, mental, appetite, and emotional conditions and report abnormal changes to nursing staff.
• Inform the RN of changes in the patient’s condition.
• Document activities and findings and submit documentation per company procedures.
• Provide post‑mortem care.

Requirements

• Valid CNA license in the State of Wisconsin.
• Valid driver’s license and auto liability insurance.
• Use standard precautions to prevent the spread of infection and disease.
• Basic computer and smartphone proficiency.
• Fluent in written and spoken English.
• Ability to work a flexible schedule, including some evenings and weekends as needed.

Pay

From $21.00 per hour.
