What You’ll Bring as Clinical Team Manager?
- Provides oversight of all patient care services and personnel
- Plans, directs, and participates in delivery of home health services
- Safeguards the timely start of patient care within recommended regulatory guidelines
- Processes and coordinates physician orders, staff assignments, and communications
- Reviews and approves plan of care and evaluates proposed changes to the plan of care
- Integrates orders from all relevant physicians involved in the plan of care
- Conducts regular OASIS reviews for all patients
- Makes regular meetings with direct care team members
- Communicates guidance, coaching, and information related to specific issues or organizational changes
- Initiates hiring requests, interviews candidates, and recommends hiring of direct care staff
- Communicates frequently with the Branch Director or Administrator on key performance metrics
- Remains available for field work, as needed
What is Required?
- Aceessory degree in a Nursing related field, Bachelor preferred
- Two years of home care experience in Home Health or Hospice environment
- A year of supervisory experience required
- Current, unrestricted RN license valid for the state of work
- Quickly becomes proficient in enterprise applications such as Homecare Homebase, Workday, and has knowledge in Microsoft Office
- Able and willing to travel up to 50% as needed to support business and patient needs
- Able and willing to lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver’s license and insurance and reliable transportation to perform job tasks
