About the Role
This role will work with hospice clients to provide personal engagement and enhance quality of life.
Responsibilities
- Conducts rigorous clinical assessments of cases, evaluating diagnosis, prognosis, pharmacotherapy, and overall trajectory
- Offers structured guidance to clinicians to uphold quality standards
- Partners with clinical staff for care goal setting, prioritization, and comprehensive planning
- Oversees or assists in clinical staff performance evaluations
- Chairs or partakes in clinical case conferences, promoting interdisciplinary collaboration
- Spearheads the development and refinement of clinical protocols, guidelines, and organizational policies
- Strictly adheres to professional clinical standards and ethical guidelines
- Advocates for a customer-centric approach within the clinical team
- Engages in clinical quality assurance and performance improvement, providing leadership and expertise
- Commits to ongoing professional development through clinical workshops and participation in clinical networks or societies
- Undertakes additional tasks as assigned by the Director of Clinical Services
Requirements
- Holds a valid Colorado State Nursing License as a Registered Nurse
- Minimum of three years of clinical nursing experience, specializing in end-of-life care
- Prior experience in clinical leadership or supervisory roles (required)
- Prior experience with CHAP, ACHC, or JCAHO (required)
- Capable of self-direction and autonomous work
- Skilled in clinical coordination and communication

Pay
The pay range for this role in the state of Colorado typically falls between $110,000 - $125,000 with the potential for performance-based bonuses and other benefits.
