About the Role

This role will work with hospice clients to provide personal engagement and enhance quality of life.

Responsibilities

  • Conducts rigorous clinical assessments of cases, evaluating diagnosis, prognosis, pharmacotherapy, and overall trajectory
  • Offers structured guidance to clinicians to uphold quality standards
  • Partners with clinical staff for care goal setting, prioritization, and comprehensive planning
  • Oversees or assists in clinical staff performance evaluations
  • Chairs or partakes in clinical case conferences, promoting interdisciplinary collaboration
  • Spearheads the development and refinement of clinical protocols, guidelines, and organizational policies
  • Strictly adheres to professional clinical standards and ethical guidelines
  • Advocates for a customer-centric approach within the clinical team
  • Engages in clinical quality assurance and performance improvement, providing leadership and expertise
  • Commits to ongoing professional development through clinical workshops and participation in clinical networks or societies
  • Undertakes additional tasks as assigned by the Director of Clinical Services

Requirements

  • Holds a valid Colorado State Nursing License as a Registered Nurse
  • Minimum of three years of clinical nursing experience, specializing in end-of-life care
  • Prior experience in clinical leadership or supervisory roles (required)
  • Prior experience with CHAP, ACHC, or JCAHO (required)
  • Capable of self-direction and autonomous work
  • Skilled in clinical coordination and communication

Pay

The pay range for this role in the state of Colorado typically falls between $110,000 - $125,000 with the potential for performance-based bonuses and other benefits.
