Position Profile: This role will work with hospice clients to provide personal engagement and enhance quality of life. Clinical Qualifications: Holds a valid Colorado State Nursing License as a Registered NurseHas a minimum of three years of clinical nursing experience, specializing in end-of-life carePrior experience in clinical leadership or supervisory roles REQUIREDPrior experience with CHAP, ACHC or JCAHO REQUIREDCapable of self-direction and autonomous workSkilled in clinical coordination and communication Clinical Job Function/Responsibilities: Conducts rigorous clinical assessments of cases, evaluating diagnosis, prognosis, pharmacotherapy, and overall trajectoryOffers structured guidance to clinicians to uphold quality standardsPartners with clinical staff for care goal setting, prioritization, and comprehensive planningOversees or assists in clinical staff performance evaluationsChairs or partakes in clinical case conferences, promoting interdisciplinary collaborationSpearheads the development and refinement of clinical protocols, guidelines, and organizational policiesStrictly adheres to professional clinical standards and ethical guidelinesAdvocates for a customer-centric approach within the clinical teamEngages in clinical quality assurance and performance improvement, providing leadership and expertiseCommits to ongoing professional development through clinical workshops and participation in clinical networks or societiesUndertakes additional tasks as assigned by the Director of Clinical Services Why Bloom? The pay range for this role in the state of Colorado typically falls between $110,000 - $125,000 with the potential for performance-based bonuses and other benefits.
