Hospice Clinical Manager

Responsibilities

  • Receives case referrals.
  • Reviews available patient information related to case, including home visits, to determine hospice needs.
  • Sets up appropriate hospice personnel for cases as needed.
  • Convenes with medical director regarding any questions about an individual's eligibility for services.
  • Evaluates each case through various means like home visits, conferences, record reviews, and services provided by clinicians.
  • Discusses and verifies impressions, instructs, and guides clinicians to enhance performance and delivery of quality hospice services.
  • Available during operating hours to assist clinicians as required.
  • Aids clinicians in establishing immediate and long-term therapeutic goals, prioritizing tasks, and creating a plan of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discuss interdisciplinary group involvement.
  • Aids Chief Clinical Officer in planning, implementing, and evaluating in-service and continuing education programs.
  • Aids in formulating, revising, implementing, and evaluating the organization’s policies, procedures, goals, and objectives, both short and long term.

Qualifications

  • Current NH RN license.
  • Previous experience in hospice/home care setting and with two (2) years management or supervisory experience.
  • Proven ability to work within an interdisciplinary setting.
  • Complies with accepted professional standards and practices.
  • Excellent observation, good nursing judgment, and communication skills.
  • Understanding of hospice philosophy and issues of death/dying.
  • Proof of Flu vaccination.
  • Reliable transportation.
  • Valid driver's license.
  • Automobile insurance.
  • Physical exam within the last year.
  • Two-step TB test.
