Responsibilities
- Receives case referrals.
- Reviews available patient information related to case, including home visits, to determine hospice needs.
- Sets up appropriate hospice personnel for cases as needed.
- Convenes with medical director regarding any questions about an individual's eligibility for services.
- Evaluates each case through various means like home visits, conferences, record reviews, and services provided by clinicians.
- Discusses and verifies impressions, instructs, and guides clinicians to enhance performance and delivery of quality hospice services.
- Available during operating hours to assist clinicians as required.
- Aids clinicians in establishing immediate and long-term therapeutic goals, prioritizing tasks, and creating a plan of care.
- Attends case conference meetings with hospice personnel to facilitate coordination of care and discuss interdisciplinary group involvement.
- Aids Chief Clinical Officer in planning, implementing, and evaluating in-service and continuing education programs.
- Aids in formulating, revising, implementing, and evaluating the organization’s policies, procedures, goals, and objectives, both short and long term.
Qualifications
- Current NH RN license.
- Previous experience in hospice/home care setting and with two (2) years management or supervisory experience.
- Proven ability to work within an interdisciplinary setting.
- Complies with accepted professional standards and practices.
- Excellent observation, good nursing judgment, and communication skills.
- Understanding of hospice philosophy and issues of death/dying.
- Proof of Flu vaccination.
- Reliable transportation.
- Valid driver's license.

- Physical exam within the last year.
- Two-step TB test.
