Hospice Chaplain

About the role

The Chaplain at Luminary Hospice is responsible for providing spiritual care services to patients and families/caregivers, either directly or through coordination with other spiritual counselors. The role involves ensuring spiritual assessments, facilitating the development of individualized plans of care, providing direct spiritual care, serving as a liaison, maintaining records, attending interdisciplinary meetings, and performing other related duties.

Job Responsibilities

  • Affirms spiritual assessment of patients and families/caregivers and renders appropriate services in a timely manner.
  • Participates in comprehensive assessments to meet identified spiritual needs and provides direct spiritual care.
  • Serves as a liaison and support to community chaplains and spiritual counselors.
  • Maintains records of spiritual care services and communicates with community chaplains and counselors.
  • Attends patient care conferences and interdisciplinary group meetings.
  • Provides consultation, education, and support to the interdisciplinary group on spiritual care.
  • Recruits community chaplains, spiritual counselors, and volunteers to meet patient and family/caregiver needs.
  • Plans and assists in funeral or memorial services as requested.
  • Assists in supervision of spiritual care volunteers when assigned.
  • Participates in quality assessment and performance improvement teams.

Job Qualifications

  • Graduate of an accredited seminary or school of theology with a BD, MDiv, or equivalent theological degree.
  • Experience working with death and dying, including individuals, families, and caregivers.
  • Ability to work as a member of an interdisciplinary group.
  • Comfort in an interfaith setting and ability to accept different lifestyles, cultures, beliefs, and values.
  • Ability to network with community clergy and congregations.
  • Knowledge of and commitment to hospice philosophy of care.
  • Hospice experience preferred.
  • Licensed driver with an insured vehicle in good condition.

Physical Demands

The physical demands of this job include sitting or standing frequently, lifting and moving up to 50 pounds occasionally, and performing tasks that require sitting, standing, and walking.
