About the role

3HC Home Health and Hospice is seeking a Hospice Chaplain to serve patients and their families in Craven and Jones County, NC. This PRN (as needed) position focuses on meeting spiritual and emotional needs during patients' illnesses, while also supporting staff during times of stress or crisis. The chaplain works closely with volunteer chaplains and area churches, with hours that include both scheduled shifts and on-call availability.

Qualifications

  • Master of Divinity or Master of Religious Education from an accredited seminary preferred
  • Bachelor's in a ministry-related field required
  • Three to five years of pastoral care experience preferred
  • Ordination to Pastoral Ministry and denominational endorsement preferred
  • Certification in Chaplaincy, chaplain experience, or completion of at least one unit of Clinical Pastoral Education (or willingness to complete after hire) preferred
  • Knowledge of grief process
  • Public relations and speaking skills
  • Knowledge of hospice philosophy, dying process, bereavement, and spirituality

Responsibilities

  • Provide spiritual and pastoral care to patients and family members, ensuring needs and support are provided in accordance with their ongoing wishes and beliefs
  • Offer patients and family members of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs, and values
  • Complete spiritual and pastoral assessments of patients within 5 days of admission for both home hospice and inpatient hospice facility patients
  • Document spiritual counseling in the patient's clinical record or EMR daily, demonstrating progress toward unresolved spiritual issues
  • Work in close collaboration with local clergy whenever desired by the patient
  • Conduct religious services of prayer, worship, and rituals for patients and families as appropriate
  • Assist in developing and updating the plan of care as needed
  • Participate in providing bereavement care and support to families and the community
  • Assist in facilitation of monthly bereavement support groups as needed
  • Work with congregations to develop pastoral care support for specific patients and for death and dying issues related to spiritual care
  • Collaborate with other hospice professionals to evaluate patient and family response to care
  • Be available to families during the bereavement period as requested
  • Participate in annual memorial services
  • Make chaplain bereavement visits and assessments as determined appropriate by the IDG team
  • Support staff in personal spiritual care, pastoral, and bereavement issues that may affect their ability to function effectively
  • Attend and participate in workshops, seminars, and webinars to keep abreast of current changes in rules and regulations
  • Pursue individual reading and research for professional growth
  • Recommend educational opportunities for staff and encourage participation
  • Demonstrate cost-effectiveness in the use of agency resources, monitoring waste, and proper and safe use of supplies and equipment
  • Create positive experiences for internal and external customers, displaying high degrees of courtesy, tact, and knowledge of services
  • Abide by and support the organization's Compliance Program and Code of Ethics
  • Perform other appropriate duties as assigned by supervisor to promote successful operations and future growth
