Patient Care & Case Management
- Conduct admission, pre-admission, routine, urgent, and pronouncement visits.
- Perform comprehensive physical, psychosocial, and environmental assessments.
- Develop, implement, and revise individualized patient care plans.
- Provide skilled nursing care and symptom management in accordance with hospice standards.
- Respond to patient needs, medical crises, and family concerns in a timely manner.
- Supervise home health aides and collaborate with caregivers to ensure quality patient care.
- Maintain accurate and timely clinical documentation.
Patient & Family Education
- Educate patients, families, and caregivers on disease progression, symptom management, medications, and end-of-life care.
- Teach caregivers how to safely perform tasks essential to patient care.
- Provide emotional support and guidance to patients and families throughout the hospice experience.
Interdisciplinary Collaboration
- Collaborate closely with physicians, social workers, chaplains, aides, and other members of the care team.
- Communicate changes in patient condition and coordinate appropriate interventions.
- Participate in interdisciplinary team meetings and care planning discussions.
Quality & Compliance
- Adhere to hospice, state, federal, and regulatory requirements.
- Follow infection prevention and control practices.
- Participate in quality improvement and performance initiatives.
- Ensure compliance with documentation and clinical practice standards.
Qualifications
- Required Education: Graduate of an accredited Registered Nursing program or Graduate of an accredited Licensed Practical Nursing program.
- Required Licensure: Current Georgia Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
- Valid Georgia driver's license.
- Reliable transportation and ability to travel throughout the service area.
Physical Requirements
- Ability to stand, walk, bend, lift, and assist patients as needed.
- Ability to lift and carry up to 50 pounds occasionally.
- Ability to work in a home care environment and travel between patient locations.
Schedule
- Full-time, Monday through Friday, 8:00 AM – 5:00 PM.
- Occasional on-call responsibilities based on team rotation.
