Hospice Case Manager, RN or LPN, Home Visits

Patient Care & Case Management

• Conduct admission, pre-admission, routine, urgent, and pronouncement visits.
• Perform comprehensive physical, psychosocial, and environmental assessments.
• Develop, implement, and revise individualized patient care plans.
• Provide skilled nursing care and symptom management in accordance with hospice standards.
• Respond to patient needs, medical crises, and family concerns in a timely manner.
• Supervise home health aides and collaborate with caregivers to ensure quality patient care.
• Maintain accurate and timely clinical documentation.

Patient & Family Education

• Educate patients, families, and caregivers on disease progression, symptom management, medications, and end-of-life care.
• Teach caregivers how to safely perform tasks essential to patient care.
• Provide emotional support and guidance to patients and families throughout the hospice experience.

Interdisciplinary Collaboration

• Collaborate closely with physicians, social workers, chaplains, aides, and other members of the care team.
• Communicate changes in patient condition and coordinate appropriate interventions.
• Participate in interdisciplinary team meetings and care planning discussions.

Quality & Compliance

• Adhere to hospice, state, federal, and regulatory requirements.
• Follow infection prevention and control practices.
• Participate in quality improvement and performance initiatives.
• Ensure compliance with documentation and clinical practice standards.

Qualifications

• Required Education: Graduate of an accredited Registered Nursing program or Graduate of an accredited Licensed Practical Nursing program.
• Required Licensure: Current Georgia Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
• Required Experience: Minimum of two years of nursing experience. Experience providing compassionate care to patients and families experiencing serious illness or end-of-life care preferred. Strong assessment, communication, and critical-thinking skills.
• Physical Requirements: Ability to stand, walk, bend, lift, and assist patients as needed. Ability to lift and carry up to 50 pounds occasionally. Ability to work in a home care environment and travel between patient locations.

Schedule

Full-time, Monday through Friday, 8:00 AM – 5:00 PM. Occasional on-call responsibilities based on team rotation.
