Hospice Care Registered Nurse (Night Shift)

About the role

Provides comprehensive, age-specific nursing care to a designated patient group, utilizing the nursing process of assessment, diagnosis, planning, implementation, and evaluation. Collaborates with physicians and healthcare team members to coordinate procedures and treatments, applying leadership and clinical judgment to supervise and delegate activities within the patient care team. The position involves performing related duties as needed, with a focus on patient-centered care, teamwork, and continuous quality improvement.

Responsibilities

• Conducts pre-admission screenings and initial assessments, develops individualized care plans based on patient needs.
• Administers nursing care, supervises diet, medications, activity levels, and safety in the home environment.
• Evaluates nursing services, including therapies and nutritional counseling, and supervises home health aides and caregivers.
• Recommends and monitors medical equipment and supplies, ensuring proper use and maintenance.
• Coordinates with interdisciplinary teams for additional therapies, requesting physician orders as needed.
• Participates in on-call coverage, maintains patient care plans, and refers complex issues to supervising nurses.
• Engages in quality improvement initiatives and ongoing training.

Qualifications

• Graduate of an accredited nursing program; Bachelor's degree preferred, with enrollment in a BSN program within two years and completion within five.
• Valid RN license in the relevant state, with specialized certifications as required.
• Minimum of 2 years nursing experience, preferably in med/surg, hospice, or home care, with at least one year in acute care.
• Experience in community health, hospice, or home healthcare is preferred.
• This role requires fieldwork with physical demands including standing, walking, lifting, and moving objects up to 150 lbs.
