Position Summary
Brighton Eugene - Weekend Liaison (Job Description)
- Aid weekday liaisons with referral follow-up and patient support.
- Focus on strategic marketing initiatives and KPI attainment.
- Provide consistent weekend coverage and continuity for referral sources.
- Navigate clinical conversations and answer clinical questions.
- Support the coordination of referrals across Brighton functions.
- Assist patients and families in navigating the health system.
- Be a hospice advocate and provide end-of-life care solutions.
- Identify opportunities to improve collaboration and increase access to hospice services.
Essential Duties & Responsibilities
- Perform all duties in accordance with Brighton Hospice policies, procedures, guidelines, contractual commitments, and governmental regulations.
- Work a Thursday, Friday, Saturday, Sunday, Monday schedule.
- Act as the subject matter expert on Medicare Hospice Benefit, Brighton Hospice service offerings, disease-specific clinical criteria, and evidence-based medicine.
- Take ownership of Pathway patient follow-ups, ensuring accurate and timely documentation and tracker updates.
- Coordinate and communicate patient status to health system, Brighton team, and other community/post-acute entities.
- Maintain performance metrics for referrals to ensure good standing with the Sales department.
- Support the admission process by facilitating hospice presentations, obtaining consents, and starting Pre-Admission Evaluations.
- Escalate clinical, referral management, customer service, and partnership concerns and opportunities to the Area Sales Director.
- Support patients, families, and communities who readmit to the hospital/ER by coordinating with the Brighton team.
- Create, develop, and sustain consultative relationships with referral sources, using effective customer contacts and excellent customer service skills.
- Show extensive experience in healthcare sales, particularly in home care and/or hospice.
- Market aggressively while maintaining positive industry relationships.
- Show good communication, negotiation, and public relations skills.
- Work independently and build relationships with referral sources.
- Be willing to drive with reliable transportation, valid driver's license, and auto insurance.
Pay
$70,000 - $75,000
