About the Role
St. Mary Home Care and Hospice, a member of Trinity Health at Home, is seeking compassionate Certified Nursing Assistants (CNA) to make a meaningful impact in the greater Philadelphia 5-county area. As a hospice/home health aide, you will provide paraprofessional services to patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, applicable licensure/certification requirements, and agency policies and procedures.
Responsibilities
- Follows the care plan developed by the RN.
- Reports client changes in condition to the RN, as described on the care plan.
- Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
- Assists clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making beds, and laundering clothes and bedding.
- Measures and records client’s oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as ordered.
- Performs range of motion and other simple procedures as an extension of therapy services as assigned.
- Assists clients in the self-administration of medication, as assigned by an RN and documented in the clinical record. The Home Health/Hospice Aide shall not administer medication of any kind.
Requirements
- High School diploma or GED required.
- Active C.N.A. certification in PA.
- Completion of a CNA course.
- Completion of a federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
- Three (3) years of employment as an aide in an institution, home care, or hospice setting.
- Current Driver’s license and reliable transportation to and from the work site.
- Current CPR training.
- Meets agency hiring criteria as defined in agency minimum standards (e.g., provision of professional references).
- Ability to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families.
- Responsibility for maintaining skills, learning, and adhering to agency policies and procedures on an ongoing basis.
