The Hospice Aide II position at Hospice of Cincinnati is an advanced role that supports patients and families during their final days. This position requires a combination of direct patient care and administrative duties.

Responsibilities

• Communicates effectively with others and maintains confidentiality.
• Reports patient related information in a timely manner and prioritizes work.
• Documents observations and tasks performed and works in collaboration with the health care team.
• Maintains a safe environment and complies with infection control policies.
• Performs clerical support, such as accurate data entry into the electronic medical record.
• May serve as a Unit Coordinator for assigned unit and shift.
• Provides hospice aide services across multiple settings or levels of care, including inpatient hospice units, home care/long term care/assisted living, and continuous care.
• Adapts personal care delivery to align with the environment and setting, patient acuity, and level of care requirements.
• Supports new hospice aides by reinforcing proper performance of personal care tasks, safety practices, and hospice philosophy.

Requirements

• High School Diploma or GED
• 2 years high school education in general studies
• 2 - 5 Years Of Previous Healthcare Experience
• Age 17 with work permit
• Reading and math ability appropriate to the position
• Multi-line phone skills, familiar with patient call systems and pagers
• Basic Life Support (BLS) within 30 Days Required
