Responsibilities

• Provide one-to-one care with patients in their homes.
• Follow physician orders and comply with the state's Nurse Practice Act, any applicable licensure/certification requirements, and Mercy Home Care policies and procedures.
• Enhance clinical excellence through the use of advanced, easy-to-use remote monitoring technology and 24/7 access to a Virtual Care Center RNs.
• Avoid preventable ER visits and hospitalizations for patients.

Requirements

• Completion of a federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
• Certified as CNA in state of Iowa.
• One year’s employment as an aide in an institution or home care setting.
• Mercy Home Care hiring criteria as defined in agency minimum standards.
• Function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.
• Maintain skills, learn, and adhere to agency's policies and procedures on an on-going basis.
• Current driver’s license and reliable transportation to and from work site.

Qualifications
