About the role
The Hospice Aide (HA) is responsible for providing direct care to hospice and long-term care patients in their primary place of residence. This includes improving quality of life, promoting comfort, and supporting patients and families through various stages of care.
Responsibilities
- Bathing, dressing, nail care, skin care, assist with elimination, nutritional needs, mobility transfers, and homemaking
- Maintain a clean, safe environment
- Take and record vital signs as per the care plan
- Observe changes in patient conditions and document accordingly
- Recognize and respond to emergencies appropriately
- Work in diverse social and economic situations while maintaining professional boundaries
- Provide excellent customer service and represent Keystone Hospice positively
- Adhere to agency, state, and federal policies and procedures, including HIPAA Privacy Standards
- Perform duties according to state and federal requirements for Certified Nursing Assistants (CNA) or Home Health Aides (HHA)
- Carry out duties in accordance with CPR and First Aid certifications or obtain them before starting employment
- Ensure compliance with Keystone Hospice’s vaccination policy against COVID-19, or obtain a religious or medical exemption upon hire
Requirements
- Certified by the state as a Certified Nursing Assistant (CNA), preferred or qualified Home Health Aide (HHA)
- Successful completion of Home Health/Hospice Aide competency evaluation requirements
- CPR and First Aid certification or the ability to obtain certification before starting employment
- Reliable transportation and valid vehicle insurance
Qualifications
- One year of CNA/HHA/HA experience preferred, but not required
- Hospice/home health/long-term care experience preferred
- Travel between long-term care facilities and/or place of primary residence
- Attend bi-weekly interdisciplinary team (IDT) meetings, if requested
