Hospice Aide

Job Summary

You will provide compassionate and essential direct care to our patients, significantly enhancing their comfort and quality of life during a critical time. You will be a vital source of support for both patients and their families.

Responsibilities

• Assist patients with personal care activities such as bathing, dressing, and mobility
• Provide light household tasks and companionship
• Observe and report changes in patient condition
• Maintain a clean and safe environment
• Offer emotional support in a dignified and respectful manner
• Follow the Hospice Aide care plan
• Document care in a timely, accurate, and concise manner
• Absess patient condition and status every visit and communicate observations to the Registered Nurse, Physical Therapist, Occupational Therapist, or Home Health Clinical/Office Coordinator
• Perform personal care services to and for patients in their homes
• Determine patient's needs and communicate to the case manager

Requirements

• Required High School Graduate or GED
• Current Home Health Aide Certificate issued by the California Department of Health Services
• One year recent work experience Home Health Aide
• Certified Nursing Assistant (CNA)
• American Heart Association Basic Life Support - CPR
• Driver's License: CAA
• Auto Insurance Preferred
