Job Summary
You will provide compassionate and essential direct care to our patients, significantly enhancing their comfort and quality of life during a critical time. You will be a vital source of support for both patients and their families.
Responsibilities
- Assist patients with personal care activities such as bathing, dressing, and mobility
- Provide light household tasks and companionship
- Observe and report changes in patient condition
- Maintain a clean and safe environment
- Offer emotional support in a dignified and respectful manner
- Follow the Hospice Aide care plan
- Document care in a timely, accurate, and concise manner
- Absess patient condition and status every visit and communicate observations to the Registered Nurse, Physical Therapist, Occupational Therapist, or Home Health Clinical/Office Coordinator
- Perform personal care services to and for patients in their homes
- Determine patient's needs and communicate to the case manager
Requirements
- Required High School Graduate or GED
- Current Home Health Aide Certificate issued by the California Department of Health Services
- One year recent work experience Home Health Aide
- Certified Nursing Assistant (CNA)
- American Heart Association Basic Life Support - CPR
- Driver's License: CAA
- Auto Insurance Preferred
