Job Description

Elara Caring is seeking an After‑Hours/On‑Call Registered Nurse to continue its industry‑pioneering work in delivering unparalleled hospice care. Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted‑living facilities, and other care settings.
• Offer compassionate support at the time of death and complete bereavement assessments for families and caregivers.

Requirements

• Current state license as a Registered Nurse.
• Graduate of an accredited certificate, diploma, associate’s, or baccalaureate nursing program.
• At least 1 year of experience as a nurse in a clinical care setting.
• Home health experience is preferred but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home‑health environment is preferred.
