Responsibilities
- Assumes responsibility for the effective coordination of patient/caregiver care.
- Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
- Visits patients to provide appropriate interventions, following the established plan of care and evaluating patient/caregiver responses to determine progress.
- Develops, implements, and evaluates plans of care for patients/caregivers, incorporating therapeutic, preventive, and palliative nursing actions and addressing patient/caregiver education needs.
- Evaluates and updates plan of care as goals and objectives are achieved or changed.
- Emergency triage by telephone and home visits.
- Provides interventions consistent with the plan of care and performs all required interventions for each visit.
- Identifies new problems or needs when they occur and facilitates patient and caregiver independence to the extent possible.
- Facilitates communication between the patient, caregivers, physicians, facility staff, and interdisciplinary team.
- Identifies and refers patient/caregiver psychosocial needs to VIA Health Partners Medical Social Worker as appropriate.
- Encourages and allows time for questions and consultations.
- Appropriately utilizes VIA Health Partners policies, patient-related protocols, standing orders, and guidelines, including Enclara Pharmacia MUGs, Medline, and ProHospice, as applicable.
- Maintains productive and professional relations with external agencies and healthcare providers.
- Maintains appropriate utilization of resources to ensure cost-effective delivery of healthcare.
- Utilizes infection control principles and ensures universal precautions in accordance with accepted professional practice.
- Keeps clinical records current by synchronizing laptop information throughout the shift.
- Completes and submits all clinical documentation per VIA Health Partners clinical documentation guidelines.
- Maintains current medication profiles as directed by physician.
- Documents medication teaching, including purpose, doses, route, frequency, actions, indication for use, side effects, and interactions.
- Records time/mileage accurately.
Qualifications
- A Diploma or Associates Degree in nursing is required; a Bachelor’s Degree is preferred.
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency and to practice in other compact states, while subject to each state’s practice laws and discipline.
- CPR (Current) is required; CHPN (Certified Hospice and Palliative Nurse) is preferred.
- Minimum 2 years related experience, including experience in direct care of patients with life-limiting illnesses, such as hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, and medical surgical.
- Prior experience in working in an environment focused on the pediatric population is required for Kids Path™ Nurse with BCLS (CPR) training for both adult and child/infant.
- Computer proficiency is required.
