Hospice After Hours

Responsibilities

• Assist the Registered Nurse in carrying out the plan of care.
• Willing to travel throughout the Bay Area for coverage.
• Avoid specialized procedures only as directed by the Registered Nurse under physician’s orders.
• Implement and evaluate the patient plan of care actively.
• Teach end-of-life care to patients and family members.
• Participate in team and staff conferences to foster cooperation.
• Participate in in-service programs and other job learning opportunities.
• Prepare clinical and progress notes as appropriate.
• Inform the Case Manager of any changes noted during visits.
• Perform on-call duties as per policy and agency needs.
• Strive for personal and professional growth through education and participation in meetings.

Requirements

• Current and unrestricted LVN license.
• One year of Hospice Experience required.
• One year of Home Health experience preferred.
• Current AHA CPR certification.
• Valid Driver's License.
• Able to communicate effectively, both verbally and in writing.
