About the role
The Admissions Nurse will be responsible for completing an initial assessment of patient and family to determine home care needs and identifying problems for each patient upon admission. This role involves outlining the patient/family referral and admission process, using assessment data to determine care plan needs and initiating the plan of care, articulating hospice philosophy, goals and objectives, communicating with other members of the clinical team to coordinate the care plan, and marketing to acute hospitals in the area.
Responsibilities

- Uses assessment data to determine care plan needs and initiates the plan of care
- Articulates hospice philosophy, goals and objectives
- Communicates with other members of the clinical team to coordinate the care plan
- Markets to acute hospitals in the area
Qualifications
- Associate's Degree in Nursing required
- Bachelor's Degree preferred
- 2-4 years of related Hospice experience
- Licensed as a Registered Nurse within the state of operations
