Hospice Admissions Nurse- RN
Capital City Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our Hospice Admissions team in Central Ohio. This full-time role focuses on conducting pre-admission assessments, educating patients and families, and initiating hospice care services.
About the role
This position involves 3 12-hour shifts, including every other weekend. The ideal candidate is patient-focused, clinically strong, and committed to delivering high-quality end-of-life care in a supportive, mission-driven environment.
Responsibilities
- Perform pre-admission evaluations to determine eligibility and appropriateness for hospice services
- Educate patients and families on hospice philosophy, care options, and services
- Initiate admission process and obtain informed consent
- Conduct comprehensive nursing assessments
- Develop and implement individualized patient-centered care plans
- Provide interventions that support comfort, dignity, and quality of life
- Collaborate with interdisciplinary care team
- Ensure compliance with quality standards and regulatory requirements
Requirements
- Graduate of an accredited Registered Nursing (RN) program
- Active Registered Nurse (RN) license in the state of practice
- Current CPR certification
- Ability to make independent clinical decisions
- Strong communication and interpersonal skills
- Ability to work effectively with patients, families, and healthcare professionals
- Knowledge of quality assurance and performance improvement processes
Preferred Qualifications
- Minimum 1 year of hospice or home health experience
- Experience with patient admissions and care planning
