We are seeking a compassionate RN Hospice Admission Nurse to perform admissions and patient assessments, primarily serving patients throughout Pierce County.

Responsibilities

• Evaluate patient eligibility, review medical histories, coordinate with the care team, and provide education and emotional support to patients and families.
• Care is provided in private homes, adult family homes, assisted living communities, and skilled nursing facilities.
• This position will be based in Pierce County, with occasional travel to Kitsap, Thurston, and King counties as needed.
• Conduct patient assessments addressing physical, emotional, and spiritual needs.
• Collaborate with physicians and the interdisciplinary team on care plans.
• Complete required documentation accurately and promptly.
• Perform hospice admissions and, when not scheduled, provide routine visits including death visits.
• Stay current with hospice best practices, policies, and regulatory standards.

Requirements

• Active Washington State RN license and BLS certification.
• Valid driver’s license and reliable transportation.
• Minimum 1 year hospice or home health nursing experience preferred.
• Minimum 2–3 years healthcare-related experience required.
• Proficient with EMR systems and healthcare technologies.
• Strong judgment, initiative, and ability to work independently.
• Culturally sensitive and adaptable to diverse patient population.
