About the role

Thresholds Homeless Outreach Program (HOP), established in 1989, provides assertive outreach, linkage to primary and psychiatric care, and resource referral to people experiencing homelessness with serious mental illnesses. As Chicago’s only citywide mental health homeless outreach program, our primary goal is to help individuals living with mental illnesses secure stable housing and support their recovery. Homeless Outreach Workers work in pairs to meet clients on the streets, in shelters, on the CTA, and in other locations where people experiencing homelessness may be found.

You will build relationships with potential and current clients to assess their needs and determine how Thresholds services or other providers can assist. In partnership with the client, you will develop a service plan with goals related to housing, benefits, treatment, advocacy, and other identified needs. In addition to long-term goals, you will address immediate needs such as food, water, harm reduction kits, hygiene items, and warming/cooling supplies.

This role requires working in the community in all weather and conditions, demanding commitment and resilience. Thresholds fosters a collaborative and supportive culture, providing tools and resources for optimal client care. Opportunities for training and professional development, including clinical supervision, will deepen your expertise in harm reduction, trauma-informed care, and other evidence-based practices.

Responsibilities

  • Conduct assertive outreach to individuals experiencing homelessness in various community settings (streets, shelters, CTA, etc.).
  • Build and maintain relationships with clients to assess their needs and connect them with appropriate services.
  • Develop and implement individualized service plans in partnership with clients, focusing on housing, benefits, treatment, advocacy, and other needs.
  • Address immediate client needs, including distributing food, water, harm reduction kits, hygiene items, and warming/cooling supplies.
  • Work collaboratively in pairs and within a supportive team environment.
  • Provide transportation to clients as needed.

Requirements

  • Passion for housing advocacy.
  • Willingness to work in the community in all weather and conditions.
  • Excellent communication and relationship-building skills.
  • Strong writing skills.
  • Good time management, organization, and flexibility.
  • At least 23 years old.
  • Valid driver’s license.
  • Willingness to provide transportation to clients.

Qualifications

Many education and experience paths are eligible for this role:

  • High school diploma or GED certificate and 5 years of supervised clinical experience.
  • Bachelor’s degree in Psychology, Social Work, or related field.
  • Bachelor’s degree in an unrelated field and 2 years of supervised clinical experience.
  • Master’s degree in Psychology, Social Work, or related field.

Schedule

Monday - Friday, 12:30pm - 9:00pm.

Pay

$24.60 - $28.27 hourly / $51,168 - $58,800 annually, based on education and experience.
