Homecare Registered Nurse

About the role

Provides primary nursing care to home based care patients as ordered by the physician, and in accordance with the organization’s policies and procedures.

Responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager.

Supports and supervises Home Healthcare Aides and LPNs as needed.

Delivers therapeutic intervention, overall care management, and achieves top decile client outcomes and patient satisfaction.

Responsibilities

• Provides one-to-one, compassionate care in patients' homes.
• Works with the Population Health Case Manager to develop care plans.
• Supervises and supports Home Healthcare Aides and LPNs.
• Ensures therapeutic interventions and care management meet patient needs.
• Achieves top decile client outcomes and high patient satisfaction.

Requirements

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the state of Delaware.
• One (1) year experience as a professional care nurse.
• Acute Care or Home Care experience preferred.

Qualifications

• Current Driver’s license and reliable transportation.
• The ability to work a weekend rotation once a month.
• The ability to work 2 holidays yearly (1 Summer & 1 Winter).
• No on-call duties.

• Strong communication skills.
• Ability to work independently and as part of a team.
• Knowledge of home care protocols and standards.
