Homecare Hospice CNA PRN

About the role
This position provides personal care and home management tasks at the patient’s residence, under the supervision of a RN Case Manager. Employees share that this role allows for meaningful one-on-one care with patients and families, fostering close relationships and providing respectful, compassionate support during end-of-life care.

Responsibilities
- Follows the nurse aide care plan to provide compassionate care in personal hygiene, elimination, feeding, and mobility.
- Reports changes in patient’s condition, concerns, or family issues to nursing staff promptly and participates in interdisciplinary team conferences.
- Collaborates with the interdisciplinary team (IDG) and other departments to achieve patient care goals.
- Documents patient care clearly, completely, concisely, and in a timely manner according to agency policy.
- Manages time, people, medical supplies, and technology professionally.
- Establishes and adheres to visit timeframes in coordination with patients, families, and facility staff.
- Maintains a clean, safe, and comfortable environment for the patient.
- Teaches or reinforces home safety, energy conservation, and sanitation.
- Provides light housekeeping tasks and assists with meal preparation.
- Demonstrates excellent communication skills and builds strong interpersonal relationships with patients and families.
- Complies with organizational nurse aide standards and participates in staff training and education programs.
- Averages 4-6 visits per day.
Requirements
- Compassionate, empathetic, and dedicated with a focus on patient dignity.
- Strong team player with exceptional bath-giving skills.
- Ability to handle heavy lifting (minimum 40 lbs).
- Maintains composure in challenging situations (e.g., dealing with bodily fluids, grieving families).
- High school diploma or equivalent required.
- 1 year of experience as a nurse aide (CNA).
- Valid CNA certification and auto liability insurance with a driver’s license.
Schedule
PRN, with a minimum requirement of 2 shifts in a 4-week period. Shifts are flexible and can occur any time Monday through Sunday.
