Homecare Certified Nursing Assistant

Part-time, day shift position located primarily at the St. Joseph PACE facility, which ministers exclusively to the elderly through a coordinated care model providing preventive, primary, acute, and long-term care services.

About the role

This role supports older individuals to live as independently as possible within their community by assisting with daily living activities, physical activity, behavioral supervision, and general activities.

Responsibilities

• Provide personal care assistance to maintain participant independence in activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in maintaining physical activity, exercise, and walking
• Provide behavioral supervision or support, such as reality orientation, medication reminders, diet restrictions, and emotional reassurance
• Assist with general activities including conversing with participants, recreation activities, meal and snack serving, and housekeeping

Requirements

• Active Certified Nursing Assistant (CNA) certification in the state where the PACE organization is located
• Minimum of one (1) year of documented experience working with a frail or elderly population (strongly preferred)
• Valid driver’s license
• Superior written and verbal communication skills
• Customer-focused with the ability to exercise independent judgment and adapt to ongoing change
• Proficiency with Microsoft Office (Word, Excel, PowerPoint) and basic knowledge of email and calendaring systems
• Position may require 10% local travel and use of a personal vehicle
• Ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, and bending
• Must be able to push, pull, move, or lift a minimum of 20 pounds
