About the role

The Children & Family Health Institute (CFHI) at VNA Health Group is seeking a dedicated Home Visitor RN to deliver essential care and support through the Nurse-Family Partnership (NFP) program in Middlesex County, New Jersey.

Responsibilities

• Conduct in-home visits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients
• Assess and address the health and developmental needs of women and their families
• Offer counseling and instruction to help families achieve targeted health and life goals
• Build and maintain strong community relationships to facilitate client referrals and resource connections
• Attend weekly team meetings, case conferences, and clinical supervision sessions
• Mentor and precept new hires and support student nurses during home visits as needed
• Ensure compliance with agency infection control and safety policies
• Maintain professionalism in adherence to organizational policies on absenteeism and appearance

Qualifications

• Associate's degree in Nursing required
• 2-3 years of relevant nursing experience; home visiting or maternal/child health experience preferred
• Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities
• Bilingual (English and Spanish-speaking) preferred

Benefits

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $42.00 to USD $50.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.

Pay

This position is covered by a collective bargaining agreement and is represented by a Union. The compensation range for this position is USD $42.00 to USD $50.00.
