Home Therapy Clinical Manager

About the Role

Responsible for the day-to-day program operations and supporting the growth of the assigned single site Home Therapies program in coordination with the Business Unit management, in-center staff, and other support functions. Ensures provision of quality patient care in accordance with policies, procedures, and training.

Responsibilities

• Provides safe and effective oversight, support, training, and scheduling of the delivery of home dialysis therapies to patients in compliance with standards outlined in facility policy procedure manuals and regulations set forth by the corporation, state, and federal agencies.
• Manages profit and loss for the assigned program, ensuring optimal performance to achieve or exceed budgets and key performance indicators.
• Directs and manages all aspects of patient care for Home Therapies patients from admission through discharge.
• Provides direction, guidance, and support to staff to ensure safe and effective delivery of home therapy support care to patients with End-Stage Renal Disease (ESRD).
• Oversees patient and caregiver readiness and ability to perform dialysis treatments at home, including assessing the home environment for suitability and training on equipment operation and maintenance.
• Manages patient experience efforts by supporting company patient satisfaction initiatives and embodying company values.
• Ensures staff competency in operating home dialysis therapy-related equipment, troubleshooting, maintaining licensures, and CPR certifications.
• Manages program supplies and equipment tracking, ensuring timely replenishment of orders.
• Directs activities related to Continuous Quality Assurance Improvement, patient retention, Clinical Algorithm Adoption, patient access management, and training success.
• Oversees data entry activities, including patient admission, regulatory reporting, billing, physician orders, lab results, vital signs, and patient status.
• Ensures regulatory, compliance, and audit activities are completed on time.
• Orients and supports new staff, providing ongoing education and ensuring compliance with risk management initiatives.
• Participates in Business Unit initiatives, projects, and audits as directed.
• Performs other related duties as assigned.

Requirements

• Graduate of an accredited School of Nursing (R.N.).
• Current appropriate state licensure.
• Successfully complete CPR Certification.
• Minimum of one year dialysis experience with three months peritoneal experience required.
• One year medical-surgical nursing or ICU experience preferred.
• Supervisory experience preferred.
• Successfully complete a training course in the theory and practice of dialysis therapies.
• Demonstrated leadership skills, including communication, customer service, performance management, and continuous quality improvement.

Physical Demands and Working Conditions

• Work environment is characteristic of a healthcare facility with air temperature control and moderate noise levels.
• May be exposed to infectious and contagious diseases/materials.
• Involves direct patient care with regular heavy lifting and moving of patients, including assisting with ambulation.
• May occasionally move machines and equipment of up to 200 lbs. with assistance, and lift chemical and water solutions of up to 30 lbs. as high as 5 feet.
• Requires frequent, prolonged periods of standing and/or walking, and the ability to bend over.
• Employees are required to take the Ishihara's Color Blindness test; reasonable accommodations will be considered if needed.

Supervision

Typically supervises non-exempt staff.

The rate of pay for this position will depend on the successful candidate’s work location and qualifications, including relevant education, work experience, skills, and competencies.
