Home Provider (PAS Attendant)

Bienvivir All-Inclusive Senior Health (“Bienvivir”) is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the Program of All-Inclusive Care for the Elderly (“PACE”). PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Responsibilities

• Provide Personal Assistance Services to participants in their place of residence, including assistance with removing and putting on clothing, bathing, eating, using the restroom, transferring, light housecleaning, and meal preparation
• Follow established individualized service plans for participants under the instruction and supervision of the registered nurse
• Assist family members in maximizing participant comfort through effective home health care
• Observe and report any changes in the participant’s status to the assigned supervisor
• Assume responsibility for providing effective personal assistance services
• Strictly follow the individualized service plan for participants
• Assist participants with ambulation and activities of daily living
• Provide Standby Assistance to prevent injury during activities of daily living
• Maintain a clean home environment for participants
• Establish and maintain an effective personal hygiene routine for participants
• Prepare meals for participants in accordance with prescribed diet
• Remind participants to take medications as prescribed by Physician
• Provide comfort care to participants
• Redirect behavior in a respectful manner
• Provide emotional support to participants
• Keep supervisor and management informed of any concerns
• Report any changes in the participants’ status or condition to the supervisor
• Comply with Personal Attendant Services guidelines and established infection control procedures
• Respect and follow participant rights, including the right to self-determination and confidentiality
• Complete mandatory in-service and compliance training timely
• Maintain certifications (if required)
• Use proper body mechanics and maintain a safe home environment
• Report safety hazards or barriers to safety to the supervisor
• Follow safety practices when using equipment such as the Hoyer lift, walker, wheelchair, and gait belt
• Encourage participants to participate in their own caretaking with appropriate consideration for their limitations
• Provide suggestions to improve the effectiveness of service delivery
• Cooperate with participants and family members to provide quality service
• Resolve requests and problems or refer them appropriately to the supervisor
• Follow special instructions from supervisor (if required)
• Assist with care of dental appliances, glasses, and assistive listening devices
• Assist in training new hires (e.g., on-the-job training and job shadowing)
• Report any issues with durable medical equipment to the supervisor
• Assist participants with emergencies by dialing 911
• Accept cases and assignments without bias
• Perform other duties as assigned

Requirements

• Must be at least 18 years of age and demonstrate competency to perform the tasks assigned by the supervisor
• Previous relevant experience in personal care or home health care, or in assisting frail, elderly people with performing activities of daily living
• Flexible to work seven (7) days a week (as assigned), including weekends and holidays
• Flexible to work overtime (if necessary)
• Flexible to work in different home settings, such as houses and apartments
• Required to complete training on use of Hoyer Lift within three (3) months of hire
• Required to complete at least 12 hours of mandatory in-service and compliance training annually

Skills

• Knowledge of care for people who have mental or physical limitations or disabilities
• Understanding of the geriatric population, the home environment, the aging process, and family dynamics
• Knowledge of relationships between participants, family members, and home care providers
• Ability to understand and follow instructions from the supervisor
• Ability to understand and communicate in English and Spanish (bilingual preferred)
• Flexibility in adapting to changes in participants’ schedules regarding activities of daily living
• Ability to discern a participant’s mood and needs
• Ability to plan, organize, and control the performance of tasks in the home environment
