Home Infusion Pharmacist - TPN Specialty

About the Role

As a Pharmacist at InfuCare Rx, you’ll be involved with multiple facets of the home infusion process. You will interact with insurance companies, staff members, distributors, physicians, nurses, and patients to ensure accurate and safe quality service. Your role will include resolving problems as part of the Pharmacy Team when unforeseen roadblocks arise. You will perform specific distributive and clinical pharmacy activities to provide safe and effective home infusion therapies.

Responsibilities

• Adhere to organization policies and procedures.
• Coordinate and facilitate new referrals/admissions through collaboration with nursing, customer service, and distribution.
• Review, receive, and verify all new patient orders and ensure consistency on order forms.
• Assist physicians and other healthcare professionals in planning and implementing patient care in the home to ensure safe and effective drug therapy.
• Conduct initial and ongoing assessments of patient, drug, and disease information to identify real and potential drug-related problems or needs.
• Assist in setting therapeutic objectives for drug therapy and establish parameters for monitoring safety and efficacy.
• Monitor outcomes from drug therapy by collecting, evaluating, and interpreting laboratory data, clinical data, and other pertinent patient information.
• Communicate results of therapeutic drug monitoring to physicians, nursing staff, and other appropriate parties.
• Program infusion control devices as indicated.
• Provide consultation and answer inquiries from patients, families, physicians, and other healthcare professionals regarding drug use.
• Maintain thorough and appropriate documentation in patient records of all clinical activities and communications.
• Prepare progress reports for referring physicians on a regular, periodic basis.
• Supervise, coordinate, and/or participate in the preparation of sterile admixtures and dispense pharmaceuticals and related supplies in accordance with federal, state, and local laws.
• Process and check all medications, supplies, and equipment prior to dispensing from the pharmacy.
• Maintain pharmacy records in compliance with applicable laws and regulations.
• Maintain clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars, and conferences.
• Participate in on-call rotation if applicable.
• Serve as the designated person for all sterile compounding activities, if applicable.
• Perform marketing support functions as requested.
• Participate in orientation and training programs for new and current staff.
• Engage in departmental quality assurance activities.
• Attend designated department meetings as scheduled.
• Perform other tasks or special projects as requested by management.

Requirements

• A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited college of Pharmacy.
• Currently licensed as a registered pharmacist in the state of employment.
• Willingness to obtain out-of-state pharmacist licenses as requested by InfuCare Rx Licensing Team.
• Minimum of two years of recent experience in an acute-care setting or one year of recent experience in home infusion therapy.
• Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
• Proficiency in Excel, Word, PowerPoint, order processing systems, and CPR+.
• Capability to evaluate and monitor drug therapy relative to the total needs of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
• Effective verbal and written communication skills with patients, clients, management, and other employees.
• Problem-solving skills and ability to supervise technical and support staff.
