Home Infusion Inpt Coordinator (FT, 40hrs/week) -

About the role

The JHIS Inpatient Coordinator contributes to the interdisciplinary team approach to assessing the eligibility and appropriateness of clients referred for home infusion therapies. This role works with clients, physicians, and the primary JHIS Pharmacist to identify the safest and most effective plan of care for infusion services.

Responsibilities

• Consult with other health professionals regarding infusion specialty specifics that impact clinical practice
• Participate in patient care planning communication with the interdisciplinary team regarding appropriateness of proposed therapies for individual clients
• Present information regarding proposed therapies, rights/responsibilities, and financial considerations to clients and significant others prior to therapy initiation
• Educate clients and caregivers regarding specific infusion-related procedures to ensure independence of performance and safety
• Communicate with Home Care Case Coordinators, Hospital Case Managers, physicians, Home Health Agencies, and the interdisciplinary JHIS team regarding client disposition prior to and immediately after discharge from the inpatient facility
• Perform client education in an efficient and effective manner, assessing learner barriers and preferred learning styles while fostering independence in procedure performance
• Communicate verbally and in writing with physicians regarding client status on home infusion therapy
• Attend mandatory education programs as required
• Perform associated duties as assigned by Nurse Manager or Director

Qualifications

• Graduate of accredited nursing school
• 2-3 years combined experience in Home Care or Home Infusion Therapy environment
• Chemo certification recommended
• Case management experience helpful

Requirements

• Current RN licensure in state of employment
• Provider in Basic Cardiac Life Support
• Valid driver's license
