About the role

We are seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in WellSky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes. Safe Life utilizes Andy AI, which assists our clinicians with OASIS completion and submission within 24 hours.

Responsibilities

• Provide exceptional home health care to patients
• Conduct clinical assessments and manage patient cases
• Utilize WellSky Kinnser EMR for documentation and OASIS completion
• Prevent hospitalizations through proactive patient care

Requirements

• Valid Illinois RN license
• Valid CPR Certification
• Valid Driver’s License and Auto Insurance
• Minimum 1 year of home health experience
• Proficiency in WellSky Kinnser EMR
• Strong clinical assessment and communication skills
• Compassionate and patient-centered approach
