Responsibilities
- Priority patient health and help prevent hospitalization
- Provide exceptional care in the comfort of patients' homes
- Utilize Andy AI to assist with OASIS completion and submission within 24 hours
Qualifications
- Valid Illinois RN license
- Valid CPR Certification
- Valid Drivers License
- Valid Auto Insurance
- Minimum 1 year home health experience
- Proficiency in WellSky Kinnser EMR
- Strong clinical assessment and communication skills
- Compassionate and patient-centered approach
