About the role

Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in WellSky Kinnser EMR to join our 4-star team.

Responsibilities

• Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes.
• Utilize Andy AI to assist with OASIS completion and submission within 24 hours.
• Show valid Illinois RN license, CPR certification, drivers license, auto insurance.
• Have at least 1 year of home health experience and proficiency in WellSky Kinnser EMR.
• Display strong clinical assessment and communication skills.
• Show a compassionate and patient-centered approach.

• Valid Illinois RN license.

• Valid Drivers License.
• Valid Auto Insurance.
• Minimum 1 year home health experience.
• Proficiency in WellSky Kinnser EMR.
