Position Overview
Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in WellSky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes. Safe Life utilizes Andy AI, which assists our clinicians with OASIS completion and submission within 24 hours.
Responsibilities
- Provide skilled nursing care in patients' homes
- Conduct comprehensive clinical assessments
- Develop and implement individualized care plans
- Monitor patient progress and adjust care as needed
- Coordinate with interdisciplinary team to ensure continuity of care
- Educate patients and caregivers on health maintenance and disease prevention
- Document patient encounters accurately and timely in WellSky Kinnser EMR
- Utilize Andy AI to assist with OASIS completion and submission within 24 hours
Qualifications
- Valid Illinois RN license
- Valid CPR Certification
- Valid Driver's License
- Valid Auto Insurance
- Minimum 1 year home health experience
- Proficiency in WellSky Kinnser EMR
- Strong clinical assessment and communication skills
