Safe Life Home Health Care, LLC was founded in 2010. We are a professional, customer-centric, fast-paced home health agency with over 100 caring professionals. We maintain state licensure, CHAP accreditation, and Medicare certification, serving as a cornerstone in community-based healthcare in Chicago. Our multidisciplinary team includes specialists in wound-care certification, lymphedema management, and other therapy disciplines. We cover 13 counties in Illinois: Cook, DuPage, Kane, Kendall, Will, McHenry, Lake, DeKalb, Boone, Grundy, LaSalle, Lee, and Ogle.

About the Role

Provides Physical Therapy services to patients as directed by the plan of care, in line with the philosophy, objectives, and policies of the agency. Supervises Physical Therapy Assistants and ensures the implementation and follow-through of PT procedures.

Responsibilities

• Assists the physician in evaluating patients’ level of function, establishing treatment plans, and setting goals.
• Performs initial evaluations within 48 hours of referral and re-evaluations every 30 days, coordinating with the physician as needed.
• Prepares clinical and progress notes, as well as required summaries.
• Revises the plan of treatment and contacts the physician for changes in the plan of care.
• Advises and consults with the patient’s family and other healthcare staff.
• Coordinates care with the agency Director of Nursing (DON) and other staff as appropriate.
• Observes, records, and reports changes in the patient’s condition and updates the agency on status or schedule changes.
• Supervises the Physical Therapy Assistant.
• Instructs patients and/or families in the use of equipment, safe patient management, and home therapy programs.
• Communicates effectively with patients, caregivers, families, and agency staff.
• Provides quality and appropriate care, reporting issues or concerns to the DON/Administrator.
• Ensures all tasks are performed in a timely manner.
• Checks in on Physical Therapy Assistants every 4-6 patient visits.

Requirements

• Licensed Physical Therapist by the state of Illinois.
• At least one year of Home Health Care clinical experience.
• Reliable car with required proof of insurance coverage.
• Maintains annual TB test, CPR certification, and other required certifications.
• Able to read and interpret instructions related to patient care.
• Able to effectively perform and prioritize multiple functions or tasks as needed.
• Must be able to travel to prospective patients’ places of residence.

Pay

$80 per visit.
