About the role

BAYADA Home Health Care is seeking a Physical Therapist (PT) for full-time, part-time, or per diem home health visits in Oxford and surrounding areas of Southern Chester County. This role serves adult and geriatric clients on a per-visit basis in their homes. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care at home.

Responsibilities

  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
  • Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Continually assess and revise the Physical Therapy care plan, collaborating with nursing on the multidisciplinary care plan as appropriate.
  • Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independent living.
  • Accurately document observations, interventions, and evaluations pertaining to client care using a state-of-the-art touchpad tablet.

Qualifications

  • A current Pennsylvania Physical Therapist (PT) license.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Pay

Base pay ranges from $65.00 to $75.00 per point, depending on qualifications and subject to negotiation.
