Responsibilities
- Perform comprehensive speech, language, cognitive, communication, and swallowing evaluations in patients’ homes
- Develop individualized plans of care based on each patient’s needs and goals
- Provide skilled speech therapy interventions to improve communication, cognition, and swallowing function
- Educate patients, families, and caregivers on therapeutic techniques, safety, and strategies for continued progress
- Monitor patient outcomes and modify treatment plans as appropriate
- Collaborate with physicians, nurses, physical therapists, occupational therapists, and other members of the interdisciplinary care team
- Maintain timely, accurate, and compliant clinical documentation
- Help patients maximize independence, safety, and quality of life at home
Requirements
- Current Speech-Language Pathologist (SLP) license in Arizona
- Current CPR certification
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interpersonal skills
- Ability to establish trust and rapport with patients and families
- Ability to work independently in a home health setting
- Strong organizational and time-management skills
- Valid driver’s license and reliable transportation
- Commitment to compassionate, patient-centered care
Home health experience is preferred, but we welcome qualified Speech Therapists from other clinical backgrounds interested in transitioning into home health.
