Responsibilities
- Administers speech therapy to patients on an intermittent basis in their place of residence, in accordance with physician orders and plan of care under the direction and supervision of the Administrator/Clinical Manager.
- Provides a full range of Speech Language Pathology Services as ordered by physician.
- Periodically participates with all other home care personnel in patient care planning.
- Takes initial history and makes initial evaluation, performs all skilled procedures.
- Consults with physicians regarding change of treatment, writes reports to physicians regarding patient's progress.
- Completes and submits an initial evaluation, including a plan of treatment and goals, to the physician for approval.
- Ensures recertification by a physician every 60 days if further treatment is to be continued.
- Writes a progress note each visit and a reevaluation when the expected duration of treatment is reached.
- Writes a summary upon patient's discharge.
Requirements
- A Master's or Doctoral degree in Speech Language Pathology and is licensed as a Speech Language Pathologist by the state where they furnish service.
- Knowledge of medical terminology.
- Reliable transportation and valid automobile insurance coverage.
Qualifications
- Two (2) years' experience as a Speech Therapist preferred.
- Home Health experience preferred.
