Acute Home Healthcare is looking for a Home Health Speech Language Pathologist (SLP) throughout Orange, Osceola, and Seminole counties to join our team.

Requirements
- Currently licensed in the state(s) in which practicing
- At least 2 years or more of experience in home care
- Level 2 Background screening (Florida Clearinghouse)
Responsibilities
- Improve or maximize the communication of the patient
- Return the individual to optimum and productive living within the patient's capabilities
- Periodically participate with all other health care personnel in patient care planning
- Provide full range Speech-Language Pathology Services as ordered by physician
- Take initial history and make initial evaluation
- Perform all skilled procedures
- Consult with physicians regarding change of treatment
- Write reports to physicians regarding patient's progress
- Assist the physician in evaluating level of function
- Help develop the plan of care and revise as necessary
- Prepare clinical and progress notes
- Advise and consult with the family and other Agency personnel
- Participate in in-service programs
- Complete and submit assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency defined time frames
