Home Health Speech Language Pathologist - PRN

We are seeking a Home Health Speech Language Pathologist to deliver personalized speech, voice, language, hearing, and swallowing therapy in patients' homes or facilities. Responsibilities include evaluating, developing, and reviewing treatment plans; administering diagnostic tests; observing and reporting patient progress; providing patient and caregiver education; maintaining documentation; and participating in care coordination and discharge planning. Qualifications include a minimum of six months' experience, current unrestricted license, CPR certification, and reliable transportation with a valid driver’s license. Preferred Experience In Home Health Is a Plus. Skills required are professional judgment in assessments and treatments, strong communication, and adherence to confidentiality standards.
